Paperback booklet with textured mustard-yellow covers, held together with two staples along the left fold. The title "Life Story of a Church" is printed in black italic ink and centered on the front cover. Centered along the bottom margin of the front margin are the years "1877 - 1953." A thin black line creates a border around the front cover. There is some wearing along the edges, and a discolored spot along the right margin of the front cover where some tape may have been taken off. The back cover is blank and in good condition.
